Diabetes Education Program

Fort Wayne Custom Rx Diabetes Program is a new program offering diabetes education to those with newly diagnosed Type 2 Diabetes, existing Type 2 Diabetes, or impaired glucose tolerance. This program is designed to help you learn the “basics” about managing your diabetes through hands on, interactive learning. This includes balancing food, exercise, and medication. If you already have diabetes, this program may help you to “get back on track.”

Our diabetes program includes the following:

Blood glucose monitoring with a new meter

Overview of diabetes and the importance of taking control

Healthy eating: Carbohydrates, protein, and fat

Taking medications properly

Being active, exercise, and what exercise does

Coping with having diabetes

Reducing risks from complications

The program is taught by a group of qualified professionals including a Certified Diabetes Educator/Registered Dietician, our Pharmacist and Registered Nurses.
